We do a mini version in psychiatry - I have plenty of older patients that I treat for mental health issues. I only do a mini cog on the ones that already present with memory or some kind of cognitive issue. The last person I asked to do one, forgot that she saw me at a recent appointment. Of course that raised alarm bells.
The fact that he's taken it so many times tells you that his providers are concerned that he has cognitive problems.

There is no such question on any known cognitive test. Nothing even remotely close.
