10. In the 1930s the US Navy's Mark 14 torpedo was developed

but not properly tested.
When war broke out, these torpedo detonators malfunctioned frequently.
Their magnetic detectors were designed to detonate the warhead under
the hull of enemy ships but they would not work properly and often the torpedo's
ran at a deeper depth than programmed.
Their contact detonators would also malfunction.
The Navy didn't have the funds to properly test these torpedo's during
the depression when money was tight.
Submarine captains were relieved of their command and moral plummeted.
After further testing, these deficiencies were discovered and corrected.
